EQUIPMENT PROVIDED: Our Professional Sound System. (We cannot provide risers, chairs, or music stands.)

PLEASE NOTE these requirements for pre-recorded audio:

Artist may provide MP3 or wav files on USB hard drive (PC) or USB flash drive.OR Artist may provide audio on tablet, phone, laptop, or other device equipped with a 3.5mm headphone output. (Note: For iPhone 7 and other devices that require a device-specific adapter to outfit it with a headphone output, artist must provide the adapter). Professional or consumer audio equipment with RCA, 1/4", or XLR outputs are also acceptable. CDs are acceptable but discouraged as they are an unreliable medium.

****** UNDERSTANDING AND AGREEMENT ******

Performers will be chosen based on 20th Century Club's desire to provide a wide range of performances of interest to those attending the Fair. If your act is selected, we will try to schedule you at your preferred day and time, but that may not be possible. We will confirm and inform you by email in April about your scheduled performance time.

I understand that I may not be selected.

If I am selected, I understand this is an unpaid performance.

I understand that I will follow the liability policies below and submit the proper form(s) in a suitable manner by the deadline indicated or my Act may not get scheduled or allowed to perform.

TOWN OF COLLIERVILLE

Participant Waiver of Liability and Assumption of Risk

(For Town Sponsored/Co-Sponsored events

or events held on Town property)

In consideration for being allowed to participate in the Collierville Twentieth Century Club 2026 Fair on the Square, the (“Special Event”), I hereby expressly assume all risks and hazards incidental to my participation in the Special Event and assume full responsibility for all risks of bodily harm and property damage resulting from or in connection with my participation in the Special Event, including without limitation, transportation to and from the activities relating to the Special Event. I hereby voluntarily release, waive, and forever discharge and agree to indemnify and hold harmless, the Town of Collierville, it’s elected officials, officers, employees, agents, representatives and related persons (collectively the “Town”) from any and all liabilities, claims, damages, transmittal of communicable disease, injuries and losses, including attorney’s fees and court costs, resulting from, arising out of or connected in any way with my participation in the Special Event and related activities; and further agree to indemnify, defend and hold the Town harmless from and against any and all liabilities, claims, damages, transmittal of communicable diseases, injuries and losses, including attorney’s fees and court costs, for personal injury or property damage to any person or entity resulting from or related to my participation in this Special Event.
